You received a phone-call from Harry Hotair, the Plant Manager of “Totally Tubular”. Harry Hotair stated that Totally Tubular is a manufacturer of vehicle inner-tubes, and is based in eastern Wisconsin. Harry Hotair stated that Totally Tubular manufactures 525 inner-tubes every day that the plant is open. Harry Hotair stated that its costs Totally Tubular $7.69 to make each inner-tube, and that each inner-tube is sold wholesale for $21.97.

Harry Hotair stated that Totally Tubular maintains 102 fulltime employees, all of whom work 40 hours/week. Harry Hotair stated that all 102 of his fulltime employees get 80-hours of paid vacation every year, and every employee takes their entire two weeks of vacation every year. Harry Hotair stated that Totally Tubular also maintains additional “fill in” employees, who work only when one of the fulltime employees is on vacation, sick, or medical leave, resulting in the same number of employees working at the plant every work-day. Harry Hotair also stated that there are five paid 8-hour holidays every year, where the plant is closed and employees do not work.

Harry Hotair stated that the underwriter for his workers compensation insurance carrier is conducting a review of his premiums, will be evaluating Totally Tubular’s performance using something that is called an “incident rate”, and comparing Totally Tubular’s “incident rate” to those of other businesses in similar industries. Harry Hotair requested that you assist him in calculating the incident rate for Totally Tubular for last year, and stated that Totally Tubular experienced the following injuries last year – all of which were filed under workers compensation:
