Week Six Assignment Scenario
Context: Audry was seen for evaluation at a cancer treatment center at a major university hospital. You are members of the behavioral health team, charged with developing a biopsychosocial evaluation. This should not be a treatment plan, but an assessment of the person’s current problems and level of functioning based on the information provided.
Personal Information: Audry is a 31-year old Caucasian female who has been married for 6 years. She lives in a large Midwestern city with her spouse of 4 years. They have no children.
Current Problems: Last year, Audry noticed a small lump in her breast. She did not consult a doctor about it until 4 months ago. A needle biopsy was performed, which was inconclusive, followed by an incisional biopsy last month. She comes to the treatment center after it was revealed to her that she had cancer in her right breast.
Psychosocial Data: Audry was born and raised in a small mountain town. Both parents are alive and still live there. Her mother and one aunt have a history of breast cancer. She describes a normal and happy childhood with conflicted relationships with both parents. She describes herself as being angry and rebellious, especially in her high school years. She saw a therapist for “about half a year” when she was 16, primarily for anger issues. After attending a local high school, she continued studies at and graduated from a small college in another state. She found a job after college as a probation officer, where she still works. She met her husband online and their relationship quickly grew. They have been married for 4 years and an important goal for both has been to have children. Thus far, she has not become pregnant and she is considering fertility treatments. The issue of infertility has led to significant conflict with her spouse. She has few friends and describes her social network as small and social support as low. She has pulled away from most of her friends in the past 3 years.
Appearance and Behavior: Audry is a short, overweight woman who looks about her stated age. Her dress and grooming are neat. She is very distressed, often crying for brief periods. Her mental status is normal, aside from her moments of tearfulness, which she cannot explain, except to say “it just comes out of the blue.”
Psychological Profile: Audry displays high levels of distress and anxiety. She is pessimistic about her future in light of her cancer diagnosis and infertility problems. She displays high levels of anxiety and describes being fearful of many situations. She is currently very angry, at herself and her spouse. She has low self-efficacy.
Behavioral Health Data: Audry has normal eating patterns with no significant weight loss or gain. There is no history of an eating disorder. Sleep patterns are undisturbed and she reports an average of 7-8 hours of sleep per night. She is a daily drinker of alcohol, claiming to have one to two glasses of wine every day. She denies use of other drugs and does not smoke. Her only exercise consists of taking 20-minute walks with their dogs each day.
