The purpose of this questionnaire is for you to describe the kind of person you are. When answering the questions, think about how you have tended to feel, think, and act over the past several years. To remind you of this, on the top of each page you will find the statement: "Over the past several years ..."
T (True) means that the statement is generally true for you.
F (False) means that the statement is generally not true for you.
Even if you are not entirely sure about the answer, indicate "T" or "F" for every question.
For example, for the question:

If, in fact you have been stubborn in many situations over many years, you would circle T.
If this was not true at all for you, or if you had been stubborn only in one or two situations, such as while working at a particular job, you would answer false by circling F.

SCORING INSTRUCTIONS
Specific scoring Instructions for the personality-disorders measure are given below. These items were specifically designed to have face validity in assessing Axis II of the DSM-IIIR, but clinical assessment would probably be more accurate than self-report in the case of some of the scales.
Instructions for scoring the PDQ-R:
The items for each scale are grouped together under a specific number. For example, on the first page, 301.20/4 refers to the Schizoid personality disorder; the “4” after the slash indicates that four items need to be answered “pathologically” in order to make the DSM-IIIR threshold for diagnosis of this disorder.
Tricky Point #1: Reverse scoring:
Normally, all items answered “T” (for true) are counted as “pathological,” but not all the items are scored the same way. You may have noticed that some of the items do not end with a period; these items are to be reverse-scored. So, for the first scale, Schizoid, items 1a, 1b, 2, 5b, and 6 are all reverse-scored. If someone answered “T” to these items, they would not be scored pathologically, and would not add to their schizoid score. So, in general, “T” is a pathological score, but 20% of the time, “F” will be a pathological score. Look on the score sheet (next page); each scale’s reversed items are given in parentheses after the scale name.
Tricky Point #2: Double scoring:
In general, give the respondant one point for each pathological answer. However, where there is a 1a and 1b (or a 6a and 6b or whatever), answering either item pathologically counts as one point; if both are answered pathologically, only one point is given.
Tricky Point #3: Cross-loading items:
On a few occasions, items count toward more than one scale. You will notice that sometimes, rather than a written item, you’ll see a number. This indicates that particular item counts on more than one disorder; so, several items “cross-load” on different scales, but not as much as with the Millon (MCMI-II) scale. All you do is go back to the previous scale and check the respondant’s answer, which gets counted toward whatever scale you are currently scoring. If any of this is still confusing, let me know.
