THIS IS THE ONE ASSIGNMENT I DO NOT WANT HANDED IN EARLY.

Do NOT write this or hand it in prior to Due Date. The idea is that you choose a goal, begin working on it, and then write about your progress in fulfilling it.

You must type (word process) at least ½ page (more if you can), single spaced paper discussing the health goal you wish to start and work on this quarter from the SPECIES Model on p. 6. Using the “Worksheet for Setting Goals” first (p. 13), summarize in paragraph form (no bullets) how you set your goal: (see SAMPLE)

· My specific goal is….

· I am measuring it by….

· I’m sure my goal is realistic because…

· I have broken my goal into smaller pieces by….:

· A good place for my journal/log to keep track of my goal is….

· My support system (people, strategies, etc) include….

· I will reward myself for achieving this goal by….

CONTINUATION OF DIRECTIONS FOR PART 1:

Next, answer the following questions in paragraph form (no bullets): (see SAMPLE)

· What part of the SPECIES model did you choose from?

· Why did you choose this goal?

· What 3 things do you hope to accomplish with this goal?

· On a scale of 1 to 10 (1 being “Not a priority for me” to 10 being “It’s the main priority in my life this quarter”) rank how much this goal matters to you now. Explain your answer

· What is the healthiest thing you are doing right now and do you think this will continue for the whole quarter? Explain why or why not.

· Setting new goals is all about change and making new habits. How do you feel about change and risking trying something new? (In other words, what is your ATTITUDE about setting a health goal for yourself? It’s OK to be honest!)

FOR FULL CREDIT TO BE CONSIDERED, YOU MUST ATTACH YOUR COMPLETED “WORKSHEET FOR SETTING GOALS” to this paper (found on p. 13). And PLEASE don’t forget to put your name on both sheets.

ALSO, I WILL NOT ACCEPT ANY PAPERS SENT BY EMAIL UNLESS I GIVE SPECIFIC PERMISSION

PART 2 INSTRUCTIONS WILL BE GIVEN TO YOU AFTER YOU HAND IN PART 1.

SAMPLE SHOWING A FULL PAGE. YOUR ASSIGNMENT IS ½ PAGE MINIMUM. Health Goal Paper Part 1 My specific goal is to jog/run 1 mile per day, 3 days a week in the Rec. Center in the SURC. I am measuring this goal by keeping track on a specialized calendar/table that I will create myself. This way I will have a visual to see how many days and what days specifically I have ran. This will also give me the visual and ability to pace my goal and space out the days. I am sure my goal is realistic because three days out of the week are reasonable for me and three days is realistic for my current fitness level and class schedule. Plus by having a backup plan I have no excuse not to find time for an alternate form of exercise if an issue arises. I have broken my goal into smaller pieces by laying out appropriate workout clothes and shoes so I am prepared to work out and also by the level of difficulty. As of right now I am not at the fitness level to run a solid mile without breaks so I will have to run for a few minutes then take a break with a brisk walk until I can run again. By trying to complete the weekly goals early in the week I am giving myself the opportunity to exceed my goal. My backup plan is that I have an alternate workout routine in the event of laziness, homework, or injury. My alternate is following along with one of my yoga/Pilates videos in my room incase I can’t make it to the gym. A good place for my journal/log is right on the wall next to my desk. If I keep it near my computer is will be a constant reminder and I will have to see it every day. My support system is my roommate because she is also working on personal fitness goals along with my other friend who is working out every day for her personal fitness goals as well. Therefore I will almost always have a buddy in the gym with me for encouragement and motivation. I decided to take my health goal all the way through the quarter whether the project is over or not. So to reward myself, I decided I will buy myself a new pair of jeans at the end of May or beginning of June.

I chose the Physical goal from the SPECIES model. I wanted to focus on a physical goal because after this year I have noticed my physical wellness is decreasing and I am not happy with my fitness level. I want to create good healthy habits for myself and since I already eat healthy and lack physical activity, it only made sense to make my focus increasing my physical activity. From this goal I hope to create a habit or natural routine of include exercise in my lifestyle, have an overall healthier weight and BMI, and have a goal that I work at almost daily. I would rate my goal as an 8 on a 1-10 scale because my first priority is doing well in school. My second priority is keeping in touch and regular visitations with my family. Because I am the youngest of three children, my mom is having a hard time with all her children being in college; therefore I try to make it home to Olympia at least 2-3 times a month. This leaves me to my next priority in line; increasing my weekly physical activity. The healthiest thing I am doing this quarter is making a conscious effort to eat healthier. This includes making healthy food choices and eating breakfast. I think this will last the whole quarter because even at home I tend to make healthy food choices so it is not a huge stretch for me to accomplish and maintain. I have a great attitude about my goal. It is challenging and also greatly beneficial to my overall health. I feel the only way I can grow is to take risks and challenge myself so this will be a great way to accomplish something and do something positive for my health.
