Initial Proctor Interview Script
Use this script as a starting off point to assess your practicum site and find a practice problem that may require a change plan. The practice problem should relate to population health or quality and safety in nursing practice.
Begin by introducing yourself to the preceptor and thanking them for agreeing to work with you throughout the course. Explain to the preceptor the requirements for your practice change assignment and the purpose of your interviews with them and anyone else you may interview.
Explain also, at some point during the interview, that you are required to implement at least one part of the change plan (the educational component) by Week 4 of the course and that the preceptor would have to assess your implementation.
1. What is your position and how long have you been working at this facility?
2. Describe your patient population. What community or communities do you serve?
3. What do you like about working at this facility? What do you think is working well here? What are the strengths of this facility? Name 3–5 strengths.
4. What are some challenges that you face? What organizational/procedural issues do you notice that could benefit from a policy or procedural change? Name 3–5 issues.
5. Is it OK if I speak to other nurses in the facility to gain a sense of potential issues so that I can develop a change plan?
6. Once I develop a change plan, in what way would the facility allow me to implement an educational component required for the assignment? Some possibilities include a 5–10-minute presentation at a meeting, a poster session, or a flyer I can share with other nurses and institutional stakeholders.
7. Can I speak to patients/clients, if the change plan relates to them?
This list of questions is just a starting point. You are welcome to ask additional questions specific to your topic and practice change.

XC4001 Interdisciplinary Team Communication Scenario
Directions: Read the following scenario and then answer the questions about the scenario.
During day shift, nursing students from a local community college have clinical on 3 North, a medical-surgical unit. Mrs Walden, the nursing instructor, tells Anna, her nursing student, to go to the nurses’ station to get an educational pamphlet for a patient being discharged. At the nurses’ station, Anna witnesses the following conversation between Joseph from respiratory therapy and Lynn, a registered nurse on the unit. Both are talking at the nurses’ station where fellow co-workers and patients can hear.
Lynn: Oh no, I have to call Dr Carr about the patient in room 302. I hate calling Dr Carr. He asks so many questions, and I don’t have time for this. He gets mad at me when I don’t know the answer.
Joseph: I hear you. He is difficult. Better you than me.
Lynn: He better just get over it and give me what I need for this patient. I wouldn’t have him care for me or my family.
Joseph: Yeah, he thinks he knows it all.
Anna (with a shocked expression on her face): Hello, I am Anna Carr, one of the nursing students. I need to see where the education pamphlets are for patients. Can you show me?
The 3 North nurse manager, Jane, enters and sees Anna at the nurses’ station. She says: Hi Anna. So good to see you. I just saw your dad, Dr Carr, in the elevator. He was bragging about how you were going to school to be a nurse. Have you seen him on the unit yet?
Anna: No ma’am. However, I think the nurse has to call him about a patient. Mrs Jane, can we talk in your office for a minute?

Module 1, Part 1: Interprofessional Communication (Template)
Interprofessional communication promotes collaboration, enhances patient safety, improves workflow, and helps nurses deliver high-quality, patient-centered care. It is a key factor in creating a well-functioning healthcare team that can address the complex needs of patients in a comprehensive and efficient manner.
You will apply communication skills to the analysis of practice problems to improve patient or population health.

Module 2, Part 2: Identifying a Practice Problem (Template)
In order to identify a practice problem that requires a change plan, you need to first study your practicum site facility, conduct some preliminary research, and assess the strengths and weaknesses of the organization to identify an area of improvement relevant to population health or quality and safety.
Begin by interviewing your preceptor using the interview script. Use the interview to assess the healthcare facility. If the preceptor and facility allow, you may also speak to other nurses and administrators at the facility. You are not limited by the script; if you come up with other questions relevant to the task of assessing the facility, you may use your own judgment to ask additional questions.
It may be helpful to record the interview so that you can analyze the key points or themes. Make sure you ask for permission before recording. If permission is not granted, then take detailed notes and make sure you capture all important details in the interviewees’ responses to the questions.
After analyzing interview and observation data, identify an issue related to population health or quality and safety at your practicum site.
Complete the Module 2 Assignment Template based on the problem you identified. Submit Module 1 and Module 2 assessments as two separate documents.
