General Notes: At Lease 1 Goal with 3 objectives and interventions. Objectives must be SMART (Specific, Measureable, Attainable, Relevant/realistic, Time-based). Make sure that interventions are specific to this client, and not just pasted verbatim from a treatment planning text. Treatment plan must be based on the theory you used to conceptualize the client. For example, if I use Super’s theory of career development to conceptualize my client, I would also use concepts from Super’s theory in the treatment plan. See example attached
