RN to BSN Nursing Program
NURS 420: Transitional Care (TC) Project
Background & Purpose
The elderly are living longer with multiple complex and chronic illnesses. Hospitalization can occur for management of these conditions. After a short hospital stay, many are discharged to lower acuity settings such as home, nursing home, or rehabilitation centers. Unfortunately, many patients are re-admitted back to the hospital due to challenges in transitioning between care settings.
The purpose of this assignment is to:
· Understand the transitional care process
· Identify common challenges and barriers
· Explore strategies for improving care transitions
· Reflect on your role as a BSN-prepared nurse in supporting safe and effective transitions
Project Overview
For this project, you will watch the Robert J Woods Foundation videos, take the associated 15-point quiz, interview a healthcare professional experienced in transitional care, and write a 50-point paper focused on the transitional care process.
You DO NOT have a patient for this assignment.
