Population:age 18-25 medical surgical unit

Prediction:

Implementing the multidisciplinary care team approach will lead to improved patient outcomes and satisfaction, as patients' behavioral health needs will be addressed more comprehensively.

Data Collection Plan (THINK ABOUT HOW YOU WILL KNOW THE CHANGE IS AN IMPROVEMENT):

What data/measures will be collected? Who will collect the data?

· The nursing staff will collect patient satisfaction scores.

· The incident reports will be reviewed by the nursing manager.

· Length of stay data will be obtained from the hospital database.

When will the collection of data take place? Data will be collected continuously throughout the 3- month cycle.

How will the data (measures or observations) be collected and displayed? What decisions will be made based on data?

Data will be collected electronically and reviewed weekly by the project lead and team. The data will be displayed graphically to track trends and changes in patient outcomes and satisfaction. Decisions will be made based on the data to adjust the care team approach if necessary.

Prediction: Implementing the multidisciplinary care team approach will lead to improved patient outcomes and satisfaction, as patients' behavioral health needs will be addressed more comprehensively.

· Patient satisfaction scores increased by 12% within the first month of implementation.

· The number of incidents related to behavioral health issues decreased by 20%.
