You should be thinking about the potential outcomes from the implementation of your project plan.
What are those potential outcomes and how could they be measured?
The paper is to be written in APA (7th ed.) format, including a title page, appropriate use of headings to separate the main sections of the body of the paper, proper in-text citations, and a references page.
The draft needs to include, but is not limited to, the following elements:
· The purpose of the project, including providing the Aim Statement
· What measure(s) will you use to evaluate the proposed project? Reflect on the types of measures that were reviewed in your week 5 learning resources (structure, process, outcome).
· Having identified the measure(s) that will be utilized to evaluate the project, what data can you utilize in the evaluation process?
· Provide a description of the process for measuring the effect (e.g., observation report, utilization of an anonymous survey, utilization of pre/post implementation survey, chart review, etc.)
· Explain whether you will be able to fully implement the project plan in the practice setting.

PLAN DO STUDY ACT (PDSA) FORM
Cycle #:
Start Date:9/6/23 End Date:5/1/25
( July 24, 2014 Credit to IHI Open School for Health Professionals for original form. Modified for ) ( Page 1 Revised: )
Project Title:
Effectively managing Mental Healthlillness
In a medical surgical unit
State
NJ
Objective of this Cycle:
Project Lead:
xTask-related;
Task: Internal Process
Develop a Change
Test a Change
X Implement a Change
Aim Statement (WHAT YOU ARE TRYING TO ACCOMPLISH):
· Specific- targeted population:
· Measurable- what to measure and clearly stated goal:
Achievable- brief plan to accomplish it:
· Relevant- why is it important to do now:
· Time Specific- anticipated length of cycle :
PLAN
Test/Implementation Plan (THINK ABOUT WHAT CHANGES YOU CAN MAKE THAT WILL RESULT IN IMPROVEMENT):
What change will be tested or implemented? Include how change will be conducted, who will run it,
where it will be run and when it will be run unless already noted in Aim Statement above. (If needed, include specifics on tasks, responsibilities and due dates.)
Population:age 18-25 medical surgical unit
Prediction:
Implementing the multidisciplinary care team approach will lead to improved patient outcomes and satisfaction, as patients' behavioral health needs will be addressed more comprehensively.
Data Collection Plan (THINK ABOUT HOW YOU WILL KNOW THE CHANGE IS AN IMPROVEMENT):
What data/measures will be collected? Who will collect the data?
· The nursing staff will collect patient satisfaction scores.
· The incident reports will be reviewed by the nursing manager.
· Length of stay data will be obtained from the hospital database.
When will the collection of data take place? Data will be collected continuously throughout the 3- month cycle.
How will the data (measures or observations) be collected and displayed? What decisions will be made based on data?
Data will be collected electronically and reviewed weekly by the project lead and team. The data will be displayed graphically to track trends and changes in patient outcomes and satisfaction. Decisions will be made based on the data to adjust the care team approach if necessary.
DO
Activities/Observations :
Record activities/observations that were done in addition to those listed in plan (above):
· Identified and trained multidisciplinary care team members.
· Developed standardized assessment and care plans.
· Conducted initial team meetings.
STUDY
Questions: Copy and paste Prediction from Plan above and evaluate learning. Complete analysis of the data. Insert graphic analysis whenever possible.
Prediction: Implementing the multidisciplinary care team approach will lead to improved patient outcomes and satisfaction, as patients' behavioral health needs will be addressed more comprehensively.
Learning (Comparison of questions, predictions, and analysis of data):
· Patient satisfaction scores increased by 12% within the first month of implementation.
· The number of incidents related to behavioral health issues decreased by 20%.
· Length of stay for patients with behavioral health/mental illness remained stable.
Summary (Look at your data. Did the change lead to improvement? Why or why not?):
The initial data suggests that implementing the multidisciplinary care team approach has had a positive impact on patient satisfaction and safety (Benjenk & Chen, 2018). However, further analysis and a longer observation period are needed to determine if the improvement is sustainable and if additional adjustments are required.
ACT
Describe next PDSA Cycle: Based on the learning in “Study,” what is your next test?
Continue monitoring and refining the multidisciplinary care team approach. Collect data for another 3 months to assess long-term outcomes. Consider additional training and support for team members if necessary.
