Instructions: 

For this assignment, you will analyze a quality improvement (QI) initiative in a clinical setting and reflect on how outcomes management can enhance the care provided.  

1. Select a Quality Improvement Initiative: Choose a QI project relevant to your practice area or a well-known initiative within the healthcare field (e.g., reducing hospital readmissions, improving patient handoff processes, or increasing vaccination rates).
   
   
2. Analyze the Initiative: 
   • Describe the main goals of the chosen QI initiative, including specific outcome measures used to evaluate success. 
   • Discuss the process measures that were implemented as part of the initiative and how they contributed to achieving the desired outcomes.
     
     
3. Reflection: 
   • Reflect on the importance of outcomes management in the QI process. How can effective outcomes management inform nursing practice and lead to continuous improvements in patient care? 
   • Consider how you would apply what you've learned in this assessment to your future nursing practice and leadership roles.
