Questionnaire for the research paper

Hi, I am supposed to  write a questionnaire of a research  paper for tab Big data and radical innovation and Analytical Maturity.  Please you are supposed to highlight to the research paper and send it to me as well along with the excel file. As per my understanding most of the research question will in the appendix of the paper.  I have attached you two files,

 One it said - Work on this file= Where you are supposed to work on 

Another it says - Example= Where you will see the tab i have made questionnaire. Please make sure that questionnaire should be on your words it should be from the research paper to clarify what survey or question they have taken for completing the research paper. Basically its a copy and paste. 

 And also please send the  attachment from original paper highlighting the things that you took it as questionnaire and wrote in excel so that i can explain where i got that actually from.  

 Please this is my final paper and its going to be grading so make sure you wont miss anything. He is about to publish is research paper therefore it is really needed.
