Quality Improvement Project

The improvement target  is to attract and retain more patients in a hospital's cardiac rehabilitation program.  The assignment is to take some tool or process from each phase and create a mock-up for this dataset. For the analyze phase you can use the analysis described in the blog. However, you need to recreate the analysis the author has done. You cannot just lift text or images. You will need to discuss the purpose of each tool and any limitations you think it has. For some of the phases, you may need to have to be creative in what you write. For instance, if you decide to create a charter, you will need to think about who would be likely team players in such a program.  You may also want to do a little research about cardiac rehabilitation programs.  The last part of the assignment is to critique the Six Sigma process. What do you think its limitations are? How do you think it might be misused?
