Reimbursement: Levels
of Care

• Routine home care: Routine care in the place
of residence

• Respite care: Interval of rest for primary caregiver provided to patients in an approved facility (no more than 5 days at a time)

• General or acute inpatient care: For pain control or symptom management

• Continuous care: Predominantly nursing care, provided for a minimum of 8 hours in a 24-hour period in the patient’s place of residence

• Per diem payments based on levels of
care include all services related to
hospice diagnosis 

– Exception: Physician can bill separately

– Reimbursement stops at death, but bereavement services continue for up to 

1 year 

– Hospice services at a contracted inpatient facility also included in per diem

– Hospice benefit periods: Two 90-day periods, followed by 60-day periods
