A 68 years old Caucasian Hispanic male presents to the clinic for chief complain of “I have been increasing my inhaler use because of some wheezing and feeling Short of breath for the past month. He states he also needs a refill on his albuterol because of his increased use of it. His symptoms increase in the evening and at bed time. He also takes Claritin 10 mg oral for seasonal allergies. His medication is albuterol 2 to 4 inhalations every 20 min as need it, seems he is using a lot. Pt has pulmonary function test about 5 years ago we will order another one to see the progression of disease.

Treatment

Refill albuterol 1 m dose inhaler with 2 refills and start with a long acting steroid such as fluticasone 1 mcg. Follow up in one week.

25 year old Hispanic female came to the clinic with unpleasant vaginal odor smell unpleasant foul fish like odor and whitish yellow vaginal discharge that worsen after intercourse (sex) with husband. She has history of vaginal douching. She reported a monogamous relationship, her husband is the only sexual partner with her husband of 10 years, she claims that here are no cause for sexual transmitted disease. Vaginal swabbing was performed and sent for cytology, the results is positive for wet Mount PH of 5.7 and clue cells. Treatment with Flagil( metronidazole) 500 mg oral 2 times a day for 7 days. Do not consume alcohol during the course of antibiotic therapy , finish medication, Instructions to the patient to always wipe from front to back use a mild soap when cleaning. Pt is instructed to return to the clinic for annual exam next year.
