PGAP assessment

I have uploaded the form that needs to be filled along with the sample paper to get better ideas. 

For the client, I want client to be 77 year old male who suffered from stroke 10 years ago.  He has some residual verbal deficit, and suffers from left sided paralysis. He uses cane to walk and walks independently but needs assistance from others time to time. He has history of diabetes, hypertension, arthritis. He is compliant with all his medication and takes his pills twice daily. He eats soft food as he currently do not have dentures. He follows vegetarian diet with exception of fish. He is currently living with his son and daughter in law but will return back to his home country. He has a high school diploma. He follows Buddhist religion and is happy with his life despite all the hardship he faced. He has a good family support and positive outlook. This is just the base but for other information theoretical infos that matches his age and his diagnosis like his medications and all.
