Total Points: 100. This is my topic: Hallmark: Delegation Concept Map 

It is the central topic: The 5 Rights of Delegation. 

1. Right Task. Definition:
The right task means the activity being delegated is appropriate for the patient's condition and is within the scope of practice of the person receiving the task. How it was violated under the article:
The RN supported the CNA in reinserting a GI tube. Reinserting a GI tube is a nursing judgment and assessment and should not have been delegated to an unlicensed assistive person. NSO, n.d.— This was not within the CNA's ability and training to do so.  

2.Right Circumstance. Definition:
The right circumstance means the patient's condition is stable, and the situation is appropriate for delegation. How was violated in the article:
The patient had a detached GI tube, which presented a medical issue with a serious chance of getting worse. The patient required assessment and probable provider involvement prior to the tube being reinserted. “The circumstances would not lend themselves to delegation, given that the situation was complex and involved considerable risk” (NSO, n.d.). 

3. Right Person. Definition: The right person means the nurse has delegated something to someone who has the education, competence and legal right to perform safely. How it was broken in the article:
While the CNA indicated that she had performed GI tube placement at a former duty, the RN did not verify that the CNA was competent or authorized by him/her to facilitate the insertion. This task wasn't done by the CNA; this was not the right person for the specific home health context (NSO, n.d.).  

4. Right Direction and the Role of Communication. Definition:
The right direction and communication mean the nurse gives clear instructions, expected outcomes, and specific information about the task it's performing. How the article violated them:
By means of a directive from the RN, I instructed her or the Nanny not to restart tube feeding immediately after reinsertion. However, communication with the patient's family was deficient because the daughter resumed starting feeding shortly after the CNA left. The RN did not ensure that there was no one following this and that they understood what the instructions and risks were. The fact that this was a communication breakdown led to patient harm (NSO, n.d.). 5. Right Supervision and Evaluation. Definition:
5. The right supervision and evaluation means the nurse monitors the delegated task, evaluates the patient's response, and follows up appropriately. When violated in the article:
The RN accepted the CNA's report that the tube was placed and did not actively supervise the protocol in the moment it was done or what occurred. It was several hours ahead of time before the RN evaluated the patient. At that point, the abdomen was painful with nausea and evidence of complications. The patient was subsequently found to have peritonitis secondary to misplacement of the tube (NSO, n.d.).  

Summary. These three rights of delegation demonstrate all five rights of delegation being violated in this case. The RN carried out a complicated procedure with an inappropriate individual in an unsafe environment that involved poor communication and inadequate supervision. Those failures brought devastating patient harm, legal action, and financial losses for the nurse and agency.
