📝 Step-by-Step Instructions (Simplified)
1. Pick a Concept
Choose a concept from your HESI Dashboard where you scored low (“Weak Performance Area”).
You can only use each concept once per course.
2. Choose a Client
Select a client that fits with the concept. This can be:
A real patient you’ve cared for,
A past encounter, or
A simulated patient from class/VR.
3. Fill in Client Information (SBAR Format)
S – Situation: Why is the client here? What’s going on right now?
B – Background: Past medical history, medications, relevant info.
A – Assessment: Vital signs, labs, your nursing assessment findings.
R – Recommendation: What needs to be done? What is the plan?
4. Make Your Concept Map
Download the concept map template from the “DFC Concept Map” module.
Fill in all boxes with client data.
Keep the map focused on your chosen concept (e.g., if you pick Glucose Regulation, focus on diabetes-related cues/interventions, not on perfusion unless it directly connects).
5. Work Through the Clinical Judgment Steps
Recognize cues: What stands out? (labs, VS, symptoms)
Analyze cues: What do they mean? What condition do they suggest?
Prioritize hypotheses: What’s the main problem? What’s urgent?
Generate solutions: Write SMART goals (Specific, Measurable, Achievable, Relevant, Time-bound).
Take action: Nursing interventions to reach the goals.
Evaluate outcomes: Did the goals happen? Were interventions effective?
6. Write SMART Goals Example
✅ Example: “The patient will report a pain level of 4/10 or less within 30 minutes of receiving pain medication.”
7. Give Credit to Sources
Cite all sources in APA 7th edition (both in-text and reference list).
Use your textbook, credible websites, or journal articles.
Three Concept Map Documents
Document 1: Glucose Regulation
Client Example: 56-year-old male, Type 2 Diabetes, A1C 9.5%, on metformin but not adherent.
SBAR: High blood sugar, polyuria, neuropathy.
Focus: Monitoring glucose, insulin therapy, dietary teaching, foot care.
Document 2: Perfusion
Client Example: 72-year-old female, Heart Failure, EF 30%, presenting with SOB and ankle edema.
SBAR: Low O2 sat, BNP elevated, fatigue.
Focus: Diuretics, daily weights, oxygen, cardiac diet, fluid restriction.
Document 3: Gas Exchange
Client Example: 65-year-old male, COPD exacerbation, O2 sat 86% RA, wheezing.
SBAR: Dyspnea, chronic cough, ABGs abnormal.
Focus: Oxygen therapy, bronchodilators, pursed-lip breathing, smoking cessation.
