Medication Adherence in Patients with Chronic Heart Failure
Medication adherence is critical for patients with congestive heart failure (CHF). It helps alleviate CHF symptoms, prevent hospitalization, and enhance patients' quality of life. Approximately 6 million adults in the United States have been diagnosed with heart failure, and the disease is responsible for more than 1 million new cases diagnosed a year in adults aged 55 and over. (American Heart Association, 2023) Medication nonadherence is one of the leading causes of these hospitalizations, with research revealing that as many as 50% of patients fail to adhere to their medication regimen as directed (Aremu et al., 2022). The following research proposal aims to assess the multiple-component intervention strategies to enhance medication compliance among targeted adult patients. This study aims to determine whether educational workshops, medication management, home monitoring, and behavioral support programs can improve patients' understanding and outcomes compared with baseline.
Background
CHF is one of the most common chronic disorders of the heart and is a leading cause of morbidity and mortality; it affects millions of people across the globe. Despite the advancement in medical treatments, medication nonadherence remains a pervasive problem in the management of CHF. According to Meraz (2019), nonadherence to medication regimens in patients with CHF leads to more symptoms, hospitalizations, and mortality. Studies have indicated that nonadherence to prescribed medication causes between 38% and 64% of CHF patients to be re-admitted to the hospital; this is one of the challenges that hinders effective disease management (Aremu et al., 2022). Some of the common issues that lead to nonadherence are economic issues, forgetfulness, adverse effects, and poor perception of the need to continue taking the prescribed medications. Educational, behavioral, and technical interventions may complement each other and increase medication adherence among congestive heart failure patients, reducing healthcare delivery costs.
Significance
Inappropriate management of medication adherence with congestive heart failure patients can cause a range of serious complications. Patients who fail to use the medicines as prescribed are more likely to develop complications—for instance, shortness of breath, edema, dyspnea of exertion, and other symptoms of CHF. As a result, the patient may become hospitalized. Patients with recurrent readmissions impose hefty charges on both them and the hospital. It also increases the patient's risk of other complications, such as hospital-acquired infections, pneumonia, and
Staphylococcus infections. In addition, nonadherence leads to a deterioration of the patient's ability to control the symptoms, physical activity decreases, fatigue increases, and the patient's quality of life decreases. The patient experiences psychological distress due to the burden of congestive heart failure, and they have frequent visits to healthcare facilities (Aremu et al., 2022). Nonadherence also increases healthcare costs due to increased use of healthcare services such as hospitalizations, emergency room visits, and other treatments, financially burdening the patient and the health systems. Lastly, the failure of patients with CHF to take their medications puts them at risk of increased mortality due to the worsening of CHF symptoms or complications, which may be fatal.
Problem Statement
The issue highlighted in this proposal is the low level of medication compliance among patients with CHF, resulting in high rates of hospitalization, decreased health-related quality of life, and high costs of care.
Purpose Statement
This study aims to assess the outcomes of a complex intervention regarding medication compliance, readmission rates, and overall quality of life for adults with CHF.
PICOT Question
In adult patients with chronic heart failure, how does the implementation of a multi-component intervention—consisting of educational workshops, medication management tools, home monitoring, behavioral support programs, and family involvement—compared to standard care, affect medication adherence rates, hospital readmissions, and quality of life over six months?
Conclusion
Nonadherence to prescribed medications remains one of the critical challenges affecting patients with chronic heart failure as well as the healthcare system in general. The problem is discussed in this research proposal, focusing on the necessity for a multi-faceted approach to enhance adherence rates. The combined multiple-component intervention may potentially decrease the rate of readmissions among CHF patients and improve their quality of life, thus easing the financial burden on healthcare facilities. The results of this research will extend the existing literature on CHF and help identify the best strategies for improving client compliance within clinical settings.
