Define the Practice Problem

• Falls are prevalent among adult populations in day cares.

• Older adults are at risk of falls because of mobility problems (Moreland et al., 2023).

• Falls may result in injuries, hospitalization or death.

• Approximately 1 per 4 adults 65 years and older/annually falls.

• There are numerous falls that can be avoided through adequate interventions.

Conform to National Health Goal.

• Healthy People 2030 Objective: IVP-08 (Towne et al., 2023).

• Minimize the number of fall-related deaths in older adults.

• Challenge: Prevention of injuries and promotion of safety.

• Helps the national priorities in public health.

• Meets with preventive care programs.

• Meets the health needs of the aging population.

Triple Aim Alignment.

• Enhance patient experience by being safe.

• Minimize fall-related injuries and fear.

• Improve population health outcomes (Kokko, 2022).

• Reduce hospitalization costs on healthcare.

• Encourage self-reliance in the elderly.

• Enhance quality of care provision.

Implement Benchmarking (Concept).

• Benchmarking is the comparison of performance against the standards.

• Assists in determining performance deficiencies.

• Leads quality improvement processes.

• Available of internal and external sources of data.

• Supports evidence-based decision-making

• Songs develop with time.

Implement Benchmarking (KPIs).

• Benchmark source: NDNQI or CMS Quality Net (George et al., 2025).

• KPI: Fall rate/1,000 days of the patient.

• Goal: Decrease the number of falls by half.

• Observe the extent of injury due to falls.

• Track compliance with fall risk assessment.

• Assess compliance of staff to protocols.

PDSA Plan Phase (AIM and Change).

• GOAL: Decrease the number of falls by 20% within 3 months.

• Enhance security in adult daycare environment.

• Change that is being proposed: Fall risk assessment tool.

• Adopt employee training in fall prevention.

• Implement environmental safety changes.

• Assistive devices should be used where necessary.

Plan Phase of PDSA (Prediction and Measures).

PDSA-Plan stage (Team and Timeline)

Team: Nurses, caregivers, administrators.

Include physical therapists

Involve patients and families.
