Abstract
Title (½ page)
After the title of the paper comes the introductory content (typically ½ page). The content will need to be updated from your initial proposal document. As you move to the final iteration of this document, remember anything in this document needs to be changed from future to past tense. Follow this template and only eliminate any Level 2 headings that may not be needed for your specific project discussion. Most of this content comes from your project course assignments. It is unlikely you will have any tables or graphs (thus no appendices). Everything that can be stated in words should be and a graphic avoided according to APA 7th edition. The final completed document should be in the range of 14 pages excluding the introduction (maximum 1 page), cover page, and references. Most projects do not include appendices. Follow the template format provided.
Problem Statement and Gap Analysis (½ to 1 page)
A concise statement identifying the problem/issue, why it is important to address, and alignment with strategic priorities of the organization. This leads to the gap analysis you conducted to identify why the project is appropriate for the site where it will be implemented.
Nature and Significance of the Problem (1 to 1 ½ pages)
Briefly describe the background and significance of the identified problem. This should be two separate sections. Provide a section overview in this level one heading. Followed by these level two headings:
Background
The Background section includes detailed evidence of the problem. It discusses the organization’s practice for the practice problem before implementation of the project.
Significance of the Problem
The Significance of the Practice Problem section includes incidence and/or prevalence; national data and quality/regulatory expectations; impact on patient, family, and health care system; financial implications; and pertinent cultural, health care policy, quality, safety, legal, and ethical implications. This section requires citations/references.
Overarching Aim of the Project (½ page)
Briefly describe the primary aim of the project. An aim is a statement of intent or what you hope to achieve at the end of the project. These are not the same as an objective or measurable outcome. They will come later in this document. An example is to lower the number of falls on a unit or reduce prescriptions of antibiotics unless warranted.
Framework (½ to 1 pages)
This section should include a description of the framework to address the problem and a discussion of how the framework correlates to the elements of the problem or activities/actions that will be undertaken to address the problem. The framework could be a relevant theory, which helps to understand why a problem exists based on behaviors, motivation, or decision-making processes and predict outcomes (i.e., Health Belief Model, Theory of Planned Behavior, Self-Determination Theory, etc.) or a conceptual model that illustrates relationships between core concepts in your topic (i.e., Technology Acceptance Model, Roy Adaptation Model, etc.), or if there is no clear framing of the concept then an in-depth framing of the concept from the literature.
Available Knowledge (1 to 2 ½ pages)
Succinctly summarize the current state of knowledge about the topic and the evidence-based/evidence-informed approach you selected to solving the problem. At the conclusion of the project you will update this section to include any recent publications since your proposal. No search strategy or tables are required. The search strategy is not included in this report.
Project Implementation (1 ½ to 2 ½ pages)
Start this section with a brief report of how the proposed project underwent IRB review and was found to be exempt. Describe the intervention with sufficient detail that others can reproduce your project in another geographic location or for another population. If modifications were made to the intervention/implementation from your original proposal discuss why this was necessary and how it was managed.
Measures and Measure Development (1 to 1 ½ pages)
Summarize the methods used to evaluate the effectiveness of the practice change. Identify metrics that were measured [outcome, process, balancing, financial, and countermeasures] as appropriate. Describe valid/reliable tools/instruments used to collect data, and permission to use. After a brief overview of what the reader will find, u se Level 2 headings for your measures. No graphics necessary in this section.

Outcomes Relative to Measures (1 to 1 ½ pages)
Provide outcomes, findings, and recommendations related to the problem statement. Report data for project outcomes utilizing methods/terms familiar to the organization (provide a glossary in the appendix if not typical to nursing/healthcare). Report evaluation of the effectiveness of the practice change and the extent to which desired outcome(s) were achieved. Report return on investment and/or value of investment as applicable. While it is unlikely you will need graphics (see note above), they would be in the appendix as per APA 7th edition criteria.

Outcomes Correlated with the Literature (1/2 page)
Provide a discussion here about consistency of the outcomes you observed/achieved through implementation of your project with what your review/synthesis of the literature demonstrated about the value of the defined project intervention.
Interpretation of Outcomes for the Organization (1 page)
Discuss the findings and implications for the organization. Include healthcare and nursing in general, as applicable. Report on organization plans to sustain the project now that it is completed. What key personnel, finances, and/or processes are necessary to hardwire into the system (e.g., policy/procedure changes, resource allocations, electronic health record modifications). What key indicators need ongoing monitoring? Include citations to support the findings and implications.
Conclusion (½ to 1 page)
Describe how results were disseminated to the organization. Technically, THIS DOCUMENT serves as your dissemination vehicle to the organization. If you have other forms of dissemination such as organizational based presentations, presentations at local, state, national or international events, or the like you should include a discussion of those here as well.
