In this assignment, you are going to formulate a Problem Statement, draft an adapted AIM Statement, and select a Theoretical Framework to utilize as a guide for designing a scholarly project.
Examine your current practice setting, or reflect on observations from a prior practice setting, and identify a practice problem. Examples of problems could be the absence of an evidence-based approach to care, a procedure that is not aligned with current best practices, practice approaches not aligned with current practice guidelines, or the inability for current practice approaches to achieve required performance metrics.
For the assignment, you are going to use the problem you identified and draft an approach for a proposed change.
The assignment format is that of an academic paper constructed utilizing APA guidelines, including a title page, appropriate use of headings to separate the main sections of the body of the paper, proper in-text citations, and a references page.
Note: You are required to complete the assignment using that template accessible from the link in the Assignment Resources section below.
The paper template includes the following sections:
· Practice Setting - A description of the practice setting where you identified the problem [your current or a prior practice setting].
· Description of the Problem - A description of the problem along with the information, observations, or data that support the identification of the problem. Include references that provide support for the observation of the problem in other practice settings.
· Problem Statement - A succinct problem statement; Utilize the document linked in the Assignment Resources section below as a guide.
· AIM Statement - An adapted 'Aim Statement' relevant to the identified problem.
· Include a timeframe for the improvement project process.
· Include your proposed intervention or change strategy.
· Descriptor of the practice setting where the improvement project will occur.
· Specify the Aim and desired outcome of implementing the proposed intervention.
· Framework - For this section of the assignment, you will identify and apply an explanatory framework to better understand why your identified problem exists. The purpose of this section is not to describe how to implement or evaluate change, but rather to analyze the underlying behavioral, motivational, environmental, organizational, and system-level factors contributing to the problem. You are expected to utilize AI ethically as a tool for idea generation and exploration by surveying the literature and identifying a minimum of three potential explanatory frameworks, theories, conceptual models, or literature-based concepts that may help explain the problem. After reviewing the AI-generated outputs, you will select one primary framework that best aligns with your identified practice problem and proposed intervention. You must synthesize the information obtained from the AI engagement into your own scholarly discussion using your authentic graduate-level voice. Do not copy AI-generated text, rely on AI as a scholarly source, or include AI formatting artifacts, unusual punctuation, or bullet-list writing styles. Frameworks focused primarily on implementation or evaluation, such as PDSA, Lean, Six Sigma, KTA, and RE-AIM, and EBP, such as Iowa, are not appropriate for this section. All references and supporting evidence included in your paper must be independently verified, appropriately cited, and supported by current scholarly literature with accessible full-text sources. As part of the assignment requirements, you must also submit a separate document containing your AI query prompts and all AI outputs reviewed during your exploration process.

Constructing a Problem Statement
What is a Problem Statement?
A problem statement is one or two sentences that identifies and summarizes a condition, problem, or issue that an improvement process is going to be developed to address.
However, before a problem statement can be written there needs to be an understanding of the difference between the symptoms of a problem and an actual problem.
Symptoms are typically the only indication that there is a problem. Symptoms are the shadows of underlying problems; the evidence by which a problem affects staff and makes itself known. In other words, a symptom is not a problem, but rather the outcome of an actual problem. A problem can be defined as the gap between the existing state and the desired state of a process.
Constructing Your Problem Statement
To write a problem statement, answer the following questions and develop your one or two sentence statement from the answers.

Tips for Writing a Problem Statement
· Look for the problem, not the solutions.
· Focus on one problem.
· Keep the statement to one or two sentences.
· Ensure you can distinguish between symptoms and problems. An effective method for doing this is to use the 5 Whys tool.

Tips for Introduction: It is always good to start a paper with a statistic (with a citation) on the topic to grab the reader's attention. Then, introduce the paper's topic and explain why it is important. In scholarly writing, do not write in the first person; instead, write in the third person. End the introduction paragraph with a purpose or thesis statement as your last sentence of the paragraph to introduce the main points/sections of your paper. Example: The purpose of this paper is to explore my practice problem of… (Include at least 2 main points/sections you will address in the body of your paper.) Practice Setting
This section should describe the practice setting where you identified the problem. This could be your current or a prior practice setting, ensuring sufficient detail to frame the problem's context (e.g., outpatient/inpatient, urban/rural, facility, unit type, population, staffing).
Description of the Problem
This section should describe your problem along with the information, observations, or data that prompted the identification of the problem. Clearly state how the current practice deviates from guidelines, evidence, or expected outcomes. This section requires evidence supporting the observation of the problem in other practice settings.
Problem Statement
This section should include a succinct problem statement using the linked document “Constructing a Problem Statement” provided in the assignment instructions and should act as a guide.
Aim Statement
This section should include your Aim statement relevant to the identified problem. Utilize IHI’s Aim Statement worksheet to guide your development of the elements of an adapted statement so that it is clear, measurable, and time-bound, though include a proposed intervention or change strategy. This section should be in narrative form, not a copy/paste of the worksheet.
Framework
This section should include a description of the framework for addressing the problem and a discussion of how the framework aligns with the elements of the problem or the activities/actions to be undertaken to address it. Utilize the linked document “Framework AI Query” to guide your construction of this section. The framework could be a relevant theory, which helps to understand why a problem exists based on behaviors, motivation, or decision-making processes and predict outcomes (i.e., Health Belief Model, Theory of Planned Behavior, Self-Determination Theory, etc.) or a conceptual model that illustrates relationships between core concepts in your topic (i.e., Technology Acceptance Model, Roy Adaptation Model, etc.), or if there is no clear framing of the concept then an in-depth framing of the concept from the literature. The framework should be supported with evidence. Do not utilize the Iowa, KTA, PDSA, or any other QI framework.
Conclusion
Tips: Notice the Conclusion is a Level 1 Header, centered and bolded. A conclusion section should be included in all scholarly papers to summarize the paper. It is normally one paragraph in length, succinct, and restates the main points of your paper. You should also restate or rephrase your purpose/thesis statement. In conclusion, you should not add new ideas or new information with citations; you should summarize the paper and reflect on key elements of the paper. If you need to add cited information, move that information to a previous section of your paper. You can end the conclusion by clarifying the intent and importance of the paper.
