Portfolio must include the following: Table of contents Copy of Pre/Post Assessment for reading/writing used with student Analysis of Data – Strengths/Weaknesses Lesson Plan for each day (see MAX for format, pg. 234) – Eight lessons Sample of Student’s work – Four examples Self-Reflection of teaching for each lesson (discuss strategy/strategies used) Peer Observation Reports – One report Copy of Research – 2 on reading instruction/2 on writing instruction (Summarize articles, describe the strategy used, and discuss how to implement in your classroom.) **You should have a cover page listing your name, course number, and semester information.
