Week 5: The Impact on Care Coordination by the APRN Paper

In the near future, advanced practice registered nurses will be the leading providers of care coordination in various healthcare settings. In this paper, discuss how care

coordination by APRNs can positively affect patient care in various clinical settings.

The paper should include the following information:

1. Select a health setting and describe the various ways in which an APRN could implement care coordination and care management activities in that setting.

2. Explain how information technology and electronic health records influence patient care and coordination.

3. Analyze facilitating factors as well as barriers and challenges related to the adoption of care coordination practices in healthcare settings.

4. Explain how APRN advocacy at the local, state, and federal levels influences legislative practices. What effect does this have on the future of advanced nursing practice in your home state?

The body of your paper should be a 3-4 pages long. This does NOT include the title page and reference page. Papers that are more than 4 pages long will not be accepted. Incorporate information from the course text plus a minimum of two (2) scholarly references published within the last five (5) years. Textbooks are not to be considered scholarly references.

Use current APA format to style your paper and to cite your sources. Your source(s) should be integrated into the paragraphs. Use internal citations pointing to evidence in the literature and supporting your ideas. Headings are required and are to be titled as identified above and include these four (4) areas.
