informed consent

 You will use the sample informed consent template provided in this week's course shell.  Note the sections in italics can remain as they are in the sample.  The remainder of the document you need to revise and rewrite in your own words.  We will review informed consent in earlier weeks of the course.  Two points of this assignment will be practicing your verbal informed consent within small groups during the class.  The remaining points will apply to your written informed consent submitted here.  Develop on a Microsoft word doc and then do a full copy/paste to the journal submission area.
