Using Microsoft Word, write two paragraphs total of at least 250 words each (more is acceptable) to address the following prompts:

· 1st paragraph: What are the health care entities involved in Joey's continuum of care?  Identify and describe these entities and the order in which they become involved in his lifelong continuum of care.

· 2nd paragraph: What are the information needs of each of his care providers?  What are some of the challenges involved with capturing and utilizing this required information? 

At the end of each paragraph, put in parentheses the total number of words for that paragraph in parentheses, e.g., (250)

For this week's discussion, please select a state (from the below list) that  has expanded Medicaid, and a state that  has not expanded Medicaid. 

For each state provide 1-2 reasons why the state made its decision to expand or not expand.  Please also provide the source of your information. 

You will receive 4 points for bullets for each state and another 2 points for the source of your information.
