In this journal activity you will submit a Kaltura video and objective note, demonstrating your skill in completing an objective exam.
You will use a live person volunteer, not a patient, and conduct the physical examination as a professional medical provider. During your examination you should verbalize for the evaluator what you are doing and why. “Assessing muscle strength.” “Assessing ROM.” “Assessing knee and meniscus” “Assessing for scoliosis.” Your comments are your choice, demonstrating your understanding of the assessment and the purpose.
I’m Using a 15y o boy with knee pain
Be sure to read the required readings and watch the videos assigned in the readings. Your final production video should be about 10 minutes. It should not exceed 14 minutes; so your preparation and succinctness in the process and moving the patient through the exam is important. Consider how you will divide your time as you begin to prepare and practice.
Include an appropriate musculoskeletal and peripheral vascular assessment for this specialty assessment.
Complete the Unit 8 Journal Documentation Template. There are two parts.
Part I is the physical assessment documentation you will complete. This is for your learning. See directions in the template for how to complete. Review the rubric.
Part II is the musculoskeletal and peripheral vascular video assessment with documentation. Be sure to use appropriate techniques addressed in the text.
You will have to practice several times to be sure your skills are honed, smooth, and that you are not looking at directions during assignment. Demonstrate professional communication with your patient.
· If your video does not include the required volunteer consent to participate and be videoed, or if it is over duration in length, or if the diagnostic reasoning scenario content is not completed, it will not be accepted. It is your responsibility to check this before you submit it.
· Complete the assessment and the documentation in a systematic way.
· Complete the questions at the end of the journal demonstrating clinical judgement and diagnostic reasoning.
See the Journal Template for Unit 8. (attached)
In this assignment you will formulate a systematic and defendable approach to nursing practice decisions for a patient examination, based on individual patient needs such as in a sports physical and gain expanded knowledge on clinical judgement and diagnostic reasoning via specific scenarios per the journal.

NU552 Journal Templates
Unit 8 Journal. Your documentation should align with the objective examination video for this specialty assessment. You should practice several times in preparation, memorizing steps and processes to present a smooth and accurate demonstration of skills in your submitted video. The student is to confirm volunteer verbal consent for video participation. Only adults are accepted as volunteers. Shirts on for the competency though in a clinical exam the patient would be draped and skin exposed.
Copy and paste your completed template into the text box along with embedding your Kaltura video. View the grading rubric.

Directions for Part I: Documentation Only
The patient History is normally completed on a form by the parent first, with the ROS; which is not a part of this journal exercise. It is not shown in this form. See required reading for examples. The provider would review with the parent/ patient.
· Complete the physical assessment documentation per the sports physical journal. Part I is not performed in the video submission but for your learning practice on sports physical documentation. · Place N in the Normal Column. Place AB in the Abnormal column with a comment if you choose. There does not need to be abnormal findings. If so, you must comment and act on it appropriately in the comment. · You must add comments in the heart and genitalia sections (as if for a male). This is for your learning. There is no video for this part.

In this assignment based on individual patient needs, you formulated a systematic and defendable approach to nursing practice decisions for a patient sports physical examination.
· When would you not approve or give only pending approval for the sport activities? Provide 2-3 specific examples |

|
|
||||
|
Complete two (2) scenarios. Once from each section. If you completed one of these in the discussion you may use that content for the related scenario and complete a new one from the other section below. Otherwise follow the directions and complete one from each group.
Complete one of these scenarios.
· Consider the chief complaint of knee pain or · Consider the chief complaint of shoulder pain or · Consider the chief complaint of radiating low back pain
And then one of these scenarios.
· Consider the chief complaint of fever in an 8-month-old. or · Consider the chief complaint of acne in a 15-year-old. or · Consider the chief complaint of nausea in a pregnant 17-year-old
Demonstrate clinical judgment regarding the chief complaint by including this content:Complete one of the three scenarios:
· What pertinent information collected from the history would impact your assessment? · What other systems need assessment due to the symptoms of the patient? · What would be pertinent positives or pertinent negatives found in the exam? · Include at least three differentials. · Include which diagnostics you would order based on your differentials and why. · How might a final diagnosis be determined? · Address a possible brief management plan · Apply evidence-based support for your choices; including a clinical guideline for one of your differentials |
