Scenario

Your healthcare system, SummitCare Health Network, serves a county of 360,000 residents through two hospitals and five outpatient clinics. SummitCare participates in several value-based care programs and is preparing a new Population Health Strategic Plan based solely on the areas you have studied in Weeks 1–8.

Your task is to design a population health assessment and management strategy that integrates:

• Foundations of public and population health
• Epidemiology fundamentals
• Measures of disease
• Screening, chronic disease epidemiology, and infectious disease epidemiology
• Social Determinants of Health (SDoH) and community assessment
• Population health approaches
• Population Health Management (PHM) frameworks
• Data and metrics used for PHM

This project should show the progression of your knowledge from Weeks 1–8 and demonstrate your ability to integrate these concepts into a unified real-world strategy.
