Section 1:   Grade 4.   You did a good job introducing the topic and your thesis statement about the need for a surveillance program.  To emphasize the importance of this you might include in this paragraph some statistics or at least a statement about why this is a problem.  I just found that information.  The first few sentences in the next paragraph should have been in the first paragraph.  The first paragraph clearly introduces the topic and includes a thesis statement that summarizes the main point of the essay and its importance. You have demonstrated mastery in this section of the deliverable.   

Section 2:    Grade 4.  I am giving you a 4 but you are unclear again about your population.  You mention Texas in the first paragraph.  Then you say the paper will be about rural Alabama.  Then you are back to Texas again.  This is very confusing! Clearly stated and thorough description of the preventable disease and population affected by the disease.  You have demonstrated mastery in this section of the deliverable. 

Section 3:  Grade 3.  Your discussion was only supported by one professional source.   Methods for a disease surveillance program are clearly stated and supported by evidence from at least two professional sources published within the past five years. You have demonstrated proficiency in this section of the deliverable. 

Section 4:  Grade 4.   Which organizations would be best suited for that and why? 

Section 5:  Grade 4.  Good job discussing several ways the information would be communicated.  Clearly stated and thorough description of how the program would be communicated to healthcare providers and communities. You have demonstrated mastery in this component of the deliverable.
