Confined Space Fatality Incident Review

Throughout the years, there have been a number of multiple-fatality incidents within confined spaces in the workplace. Choose one such event, and summarize what contributed to the incident. Utilize the knowledge that you have gained in this unit, as well as personal experience, to determine what could have been done to prevent the accident.

Your paper must be a minimum of one page in length. You must include a minimum of three sources, including the textbook, and one that must come from the CSU Library. All sources must be referenced; paraphrased and quoted material must have accompanying APA citations
