Form linked To data base.

Good Morning,

Am a College Student I have Assignment; I want to create a formal form that will be linked to my Database(Access , excel). 

For example, 

*Each Employee has many Furniture. 

*Each Furniture has a unique number"Tag" connected to one customer .

So I need one form to do 2 jobs at the same time  :

1- Fill form for each employee ,so I can print it and get his signature.

2- Data entered in the form saved directly to my database. 

*I want Employee Information to appear next to each Furniture tag. And Tags listed in the form,I want them to be saved (each tag in new row + employee info).
