Critical reflection of your growth and development during your practicum experience in a clinical setting has the benefit of helping you to identify opportunities for improvement in your clinical skills, while also recognizing your strengths and successes.
Use this Journal to reflect on your clinical strengths and opportunities for improvement, the progress you made, and what insights you will carry forward into your next practicum
To Prepare
· Refer to the “Advanced Nursing Practice Competencies and Guidelines” found in the Week 1 Learning Resources, and consider the quality measures or indicators advanced nursing practice nurses must possess in your specialty of interest.
· Refer to your “Clinical Skills Self-Assessment Form” you submitted in Week 1, and consider your strengths and opportunities for improvement.
· Refer to your Patient Log in Meditrek, and consider the patient activities you have experienced in your practicum experience. Reflect on your observations and experiences.
In 450–500 words, address the following:
Learning From Experiences
· Revisit the goals and objectives from your Practicum Experience Plan. Explain the degree to which you achieved each during the practicum experience.
· Reflect on the three (3) most challenging patients you encountered during the practicum experience. What was most challenging about each?
· What did you learn from this experience?

1. Goal: Enhance diagnostic reasoning skills. a. Objective: Develop and prioritize a differential diagnosis list for five patient cases under supervision by week 6. b. Objective: Formulate DSM-5-TR diagnoses based on data for three cases independently by the end of week 10. c. Objective: Differentiate between normal and abnormal symptoms in at least five patients by mid-practicum.
2. Goal: Improve proficiency in screening tools. a. Objective: Identify and choose proper psychosocial screens for common psychiatric disorders by the end of Week 5. b. Objective: Demonstrate the ability to interpret and document screening results for five cases with not less than 80% accuracy at the end of the practicum. c. Objective: The student should recommend a specialist when needed at least two instances by Week 8.
3. Goal: Strengthen collaboration with multi-disciplinary teams. a. Objective: Attend and contribute to two team meeting per month to discuss treatment plans. b. Objective: Submit one integrated case analysis to the team by Week 12. c. Objective: Implement patient care plan feedback from team meetings for two cases.
4. Goal: Promote health education and prevention strategies. a. Objective: To deliver psychoeducation on disease prevention to at least five clients or caregivers by mid-practicum. b. Objective: Write about the effects of Human health promotion techniques with the execution of normal/patient care plans in blunt by Week 10. c. Objective: Create an information sheet based on the symptoms and care advice which will be useful for patients with chronic psychiatric diseases by the end of the practicum .
