Family Interview

• Use the family needs assessment template/tool,  
• Construct an Ecomap and Genogram for your family. (See Supplemental resources for assistance) Attach these as an Appendix at the end of the post using the Family Assessment tool to complete. Make sure you add disease or health problems to the Genogram.

1. Analyze Findings Using Social Determinants of Health (SDOH): 
   •  Identify at least two social determinants that might influence the family’s health (e.g., socioeconomic status,  access to healthcare,
      education, neighborhood environment, social support).
   • Discuss how these determinants may positively or negatively affect the family’s overall health and access to care. 
     
2. Propose a Specific Community/Population-based Nursing Intervention: 
   • Develop a targeted, evidence-based nursing intervention addressing the family’s identified health needs.
   • Include:
     
     • Measurable goal/outcome of the intervention 
     • Two resources needed or referrals located near the family. Be specific.
     • How the intervention addresses the identified SDOH
3. Reflective Component: 
   • Briefly reflect on what you learned from conducting the assessment and how understanding SDOH can improve family-centered
     care in community nursing. 
     

Assignment Requirements:

• The post is to be clear and concise, and students will lose points for improper grammar, punctuation, and misspelling.
• The post is to be 5-7 pages in length (excluding references, title page and Genogram/Ecomap; an abstract is not required) 
• Incorporate a minimum of 5 current (published within the last five years) scholarly journal articles or primary legal sources (statutes, court opinions).
