Family Disaster Plan Checklist

1. Complete all sections. Use yes, check marks, or X's for items you do have. Use need or no for items you do not have Use N/A, for items not applicable for your living situation. Do not leave anything blank.
2. Remember to protect personal information by using pseudonyms, or by providing only partial information.

1. Use complete information for utility companies, public companies, and agencies.
2. Complete Family Escape Plan

• Draw your home floor plan using a home escape plan template.
• Label all the rooms and identify the doors and windows (see blue window markings)
• Plan 2 escape routes from every room (see red and yellow markings)
• Indicate a designated Meeting Place outside your residence (see tree indicating Meet Here)
• NOTE: If you live in a multi-story dwelling, explain your escape route if you cannot use the stairs to exit the upper floors.
