Evidence based plan- EBP Plan assignment, please keep the following in mind:

• fill out the two formatted tables using the direction provided below and two research article that pertains to the PICOT study. 
• This assignment is considered the final for this      course.
• Please be sure to use a FULL RESEARCH ARTICLE either      quantitative or qualitative for the document titled "formatted table      for individual study" and please be sure to use a FULL SYSTEMATIC      REVIEW ARTICLE (either quantitative or qualitative based) for the document      titled "Formatted Table for Systematic Review"
• Remember, you can use articles you've already found      in previous assignments - but not the articles from your week 11 and 12      assignments.  these articles are attached. Do not use  the research article that I have attached. 
• Fill in both documents fully, based on an article      for each related to your PICOT question. There are two separate forms. I have attached these forms. 
• This assignment - both documents - is very similar      to the Week 11 and 12 critique assignments you already completed, just in      a different format. I have attached week 11 and 12 assignments for you to    look at. 
• You must provide the reference in  APA format and also supply the articles for me to review. Without      submitting your articles, I cannot review them to appropriately grade your      assignment, forfeiting your grade. Please attach the 2 articles you are using! 
• When you're working on your EBP Plan assignment,      please keep the following in mind:
