Purpose:

To develop an evidence-based intervention plan addressing a priority area using public health data and a statewide community improvement plan.

Instructions:

Using the Florida State Health Improvement Plan (SHIP) choose priority area #7 Transmissible and Emerging Diseases
Click on the view more tab
Read the information on the page and then proceed to the data dashboard for information on specific goals
Note the guiding priority area goals. Review the page and investigate the data provided specifically the listed objectives
Investigate the following objective: TED 1.2: Reduce the rate of new HIV diagnoses among all Floridians.
Analyze the epidemiological data, including:
- Trend of new Florida HIV diagnoses. Is the SHIP goal being met?
- Click on Miami-Dade County rates per 100,000 and analyze the results in comparison to Florida for the latest year. Compare years and discuss this. What is the trend noting?
- At-risk populations: Name and discuss two for Miami-Dade County
- Identify two relevant social determinants of health impacting the high incidence and prevalence of the disease.
Develop a community health plan for Miami-Dade County that includes:
- Problem statement
- One measurable goal/objective
- One evidence-based intervention
- Two specific implementation strategies specific to Miami-Dade County
- One evaluation method specific to Miami-Dade County

The community health plan must be supported with evidence-based resources.

Assignment Requirements:

The post is to be 3-4 pages in length (excluding references, title page; an abstract is not required)
Incorporate a minimum of 3 current (published within the last five years) scholarly journal articles or primary legal sources (statutes, court opinions).
