 Re-read the hypothetical case scenario you created.  

Conduct some online research and identify community resources that would be beneficial to the client and/or family. 

 In a minimum of TWO-pages (not counting the title page and reference page) address the following:

• Identify at least four (4) community resources that may be beneficial to the client and/or family.
• Provide the name and location for the community resource and why you believe the resource would be beneficial to the client and/or family.

 Using the Sample Referral Form, complete at least three (3) referrals for the client and/or family. The only thing that I need completed on the referral form completed is the reason for the referral.
