Your survey results must be submitted here in order to receive credit.

The executive-format programs are a unique approach to higher education. With the required Applied Learning Practicum each semester, you are taking education beyond the cognitive level and learning how to apply your knowledge to the real world profession within your field of study. The purpose of this format is to provide you with the skills and knowledge necessary to leave your program at UC as job ready! 

This assignment is worth 40 points (40% of your grade). Please answer all questions thoroughly and to the best of your ability. This assignment will likely take 1-2 hours to complete. 

Once the survey has been submitted you will see an image similar to the one below. You MUST click "SAVE IMAGE" and then save the survey to your computer.  From there, you will upload the saved image into Blackboard.

If you do not submit the summary into Blackboard then you will not receive full credit for the assignment. You will also receive an emailed confirmation once the survey has been submitted.
