According to the NYC Health + Hospitals (2016), the organization's Community Health Needs Assessment (CHNA) depicts a higher rate of chronic non communicable disease such as heart disease, high blood pressure, and diabetes. These chronic non communicable diseases within the community are issues that need to be addressed. For my final project, I decided to introduce a wellness program that includes physical exercise as well as nutritional education. The program's objective is to assist patients that would like to begin a healthier lifestyle after they've been discharged from the hospital. The marketing goal for this program, would be to have a minimum of 15% of patients seen at Bellevue looking to live a healthier lifestyle, introduced and signed up for the program within six months to one year after being discharged.
Cellucci, Wiggins, & Farnsworth (2014), stated that the five P's stand for patients, physicians, payers, the public, and politics. This program will focus on both the patients and the public. The program will help patients affected by heart disease, diabetes, and high blood pressure live a healthier lifestyle. This program will also help to improve the overall health of the community and address some of the issues the community is facing.

The service that I proposed for Bellevue Hospital is a Community Response Team. The Community Response team is a service that responds to crisis and assist with the recovery of people with Mental illnesses in the Community. This service will aim to raise awareness in the Bellevue Community letting those who may experience symptoms of mental illnesses that they are not only.
The two Five Ps that I will implement into my final healthcare marketing and communication plan are public and patients. "Public health is the organized community effort aimed at the prevention of disease and the promotion of health." The service that I am marketing is not only for one individual it is to service the whole community and get the public involved and aware of mental health. There are many people in the community who may not suffer from mental illnesses but have family members and love ones that struggle mentally. Also, involving the public also involves other health facilities and local law enforcement. The Community Response team will help to educate local law enforcement officers on how to deal with people who are mentally ill. Next, patients are the center of the community response team. The community response team will help service and rehabilitate patients who suffer from mental illnesses.
