Instructions:

Select a behavior (for example, smoking, physical activity, diet, cancer screenings, etc.). Using at least 3 of the social and behavioral health theories covered in this course, describe the behavior from the perspective of each theory. For each theory, summarize the theory—including identification and explanation of each theory construct—and then apply the theory to the behavior. The conclusion should include a selected theory (from the ones described in the paper) that is best suited and applied to the selected behavior—along with support for your claims and reasonings. Students will also identify an evidence-based intervention that applies the selected theory to the behavior and describe how and why the intervention is appropriate for the theory and behavior. Additionally, students will then describe how to adapt the selected intervention and theory to serve diverse communities. Refer to “paper sections” for more specific information on paper content.

General Instructions:

1) The name of the file should include the course #, paper, and student’s last name. Example: “SPPH 5304_paper_stjohn”

2) Number pages (last name & page number in the footer)

3) 1 inch margins, 12 point font (Calibri)

4) Length: 5-10 pages (single spaced), not including references, table of contents, or title page

5) Figures, charts, graphs, etc., are allowed but there should not be more than one per page or more than 5 in the paper

6) References: Paper should have at least 5 sources. Class lectures should NOT be used; rather, use original sources (such as the text book and/or articles).

7) Proof read your paper. Grammar/punctuation will be part of the grade. (Refer to checklist). Avoid first and second voice/person.

8) Refer to the posted paper examples for ideas, etc. Note: these are examples and DO NOT necessarily mean these papers are perfect/A papers (nor have all the required information).

d. Explain what the paper will seek to do (examine the variables that may be associated with the behaviors related to the public health topic)

4) Theory 1

iii. Statement of how social and behavioral health theory is applied to health behavior change

b. Application

. Explain how the social and behavioral health theory can be applied to the public health issue (how would you operationalize the constructs, etc.)

7) Preferred Solution & Selection Intervention a. Preferred solution - Which social and behavioral health theory is best suited for and applied to this public health issue and why?

- Support your claims with evidence.

c. Describe how to adapt the selected intervention and social and behavioral health theory to serve diverse communities. - Briefly describe how to adapt the selected intervention and selected social and behavioral health theory in a way that will serve diverse communities.

- Describe the diverse communities your intervention will serve.

- Briefly discuss why selecting a particular social and behavioral health theory and adapting interventions to better serve diverse communities is important. How does this fit into the core roles and functions of public health?

8) Conclusion a. Closing paragraph - Summarize selected behavior and why you selected the behavior (consider citing data—how big of an issue is this and how does the behavior impact people?

- Summarize selected social and behavioral health theory and why you selected this theory for this behavior.

- Summarize selected intervention and why and how the intervention fits with the selected social and behavioral health theory and the selected behavior.

- Summarize recommended adaptations to the selected social and behavioral health theory and intervention in order to serve diverse populations.

9) Works cited a. Use valid, scholarly articles, books, and publications (Wikipedia is not a valid source).

b. Use online libraries resources (PubMed, etc.)

c. Use APA 7th ed. citation method & be consistent.

d. Points will be deducted for references not in proper format.
