HhacchteThis is a test you can be using on your patients every day in clinical and you should be familiar with it, as it is an easy to administer early warning system. You do not have to answer the questions in any particular order, but I included what I think is important to be included in the answer

What does it test?
Can you think of any portion of the test that might be affected by a patient who is intubated?
What are the guidelines when administering it?
How is it scored? What is the highest score?
When should the nurse notify the health care provider after completing this assessment? Why?
