 People make mistakes, but workplace errors may have far-reaching  consequences, affecting the company, coworkers, customers, or others.  These errors may have been just a missing comma in a legal document, a  missing or misplaced semicolon in code, a misspelling, a misplaced  decimal point, etc. 

• In your first paragraph, provide a summary of the incident. Citation is not required.
• In a second short paragraph, provide your reaction. This could be an  explanation of what you think could have been done to avoid the problem  or a comparison to a similar event from your workplace.
• Provide the full URL (not a text link) to your source/webpage. Use only one source.
