Her activity tolerance has significantly declined, and she now experiences dyspnea with minimal exertion, including climbing stairs and walking to her backyard.

Probable Diagnosis:

• Iron-deficiency anemia (IDA) is the most likely diagnosis based on progressive fatigue, weakness, dizziness, palpitations, exertional dyspnea, and decreased activity tolerance. These symptoms may result from reduced oxygen-carrying capacity associated with anemia and warrant further evaluation to identify the underlying cause (Burton et al., 2020).

Differential Diagnoses:

• Vitamin B12 deficiency anemia should be considered because it can cause anemia, fatigue, weakness, and neurologic symptoms such as dizziness. Vitamin B12 testing can help differentiate this from iron-deficiency anemia (Pagana et al., 2022).
