"Andreanna: History of Present Illness: A 58-year-old African-American female presents with worsening shortness of breath and palpitations for approximately one week. She reports intermittent dizziness for several months with progressively worsening weakness and fatigue over the past month. Her activity tolerance has significantly declined, and she now experiences dyspnea with minimal exertion, including climbing stairs and walking to her backyard.
Probable Diagnosis:
- Iron-deficiency anemia (IDA) is the most likely diagnosis based on progressive fatigue, weakness, dizziness, palpitations, exertional dyspnea, and decreased activity tolerance. These symptoms may result from reduced oxygen-carrying capacity associated with anemia and warrant further evaluation to identify the underlying cause (Burton et al., 2020).
Differential Diagnoses:
- Vitamin B12 deficiency anemia should be considered because it can cause anemia, fatigue, weakness, and neurologic symptoms such as dizziness. Vitamin B12 testing can help differentiate this from iron-deficiency anemia (Pagana et al., 2022).
- Anemia of chronic disease/inflammation may cause progressive fatigue, weakness, and reduced exercise tolerance and should be considered if iron studies do not support absolute iron deficiency (Burton et al., 2020).
- Heart failure should be considered because worsening exertional dyspnea, fatigue, weakness, and decreased exercise tolerance may also indicate underlying cardiac dysfunction.

Rationale
Evidence
Citation
CBC with RBC Indices
Confirm anemia and classify by RBC size.
Low Hgb/Hct with low MCV/MCH supports microcytic anemia.
(Pagana et al., 2022)
Reticulocyte Count
Assess bone marrow response to anemia.
Low/inappropriate response may indicate decreased RBC production.
(Pagana et al., 2022)
Serum Ferritin
Evaluate iron stores.
Low ferritin strongly supports iron deficiency.
(Pagana et al., 2022)
Serum Iron, TIBC & Transferrin Saturation
Differentiate IDA from other causes of anemia.
Low iron/saturation with elevated TIBC supports IDA.
(Pagana et al., 2022)
Vitamin B12 & Folate
Evaluate nutritional causes of anemia.
Deficiency may indicate megaloblastic anemia.
(Pagana et al., 2022)
Fecal Occult Blood/FIT
Evaluate for occult gastrointestinal blood loss.
Positive results warrant further GI evaluation.
(Pagana et al., 2022)
Pharmacological Management: If iron deficiency is confirmed, initiate oral iron replacement, with intravenous iron considered when oral therapy is ineffective or not tolerated. Treatment should also address the underlying cause of iron deficiency rather than iron replacement alone (Burton et al., 2020).
Nonpharmacological Management: Encourage an iron-rich diet containing lean meats, beans, leafy green vegetables, and iron-fortified foods. Activity should be adjusted based on symptoms until hemoglobin improves. If occult GI blood loss is suspected, referral for gastrointestinal evaluation should be pursued (Burton et al., 2020).
Patient Education: Educate the patient on taking iron as prescribed, potential gastrointestinal side effects, and the importance of follow-up laboratory testing. She should seek urgent evaluation for worsening shortness of breath, chest pain, syncope, or severe palpitations (Burton et al., 2020).
Follow-Up: Reassess the patient within several weeks with repeat CBC and iron studies to evaluate response to treatment. If iron-deficiency anemia or occult blood loss is confirmed, refer to gastroenterology to identify the underlying source and guide further management (Burton et al., 2020).
