Conducting a Nutrition Assessment

 For this assignment you will conduct a Nutrition Assessment and complete the Nutrition Assessment Intake Form Download Nutrition Assessment Intake Form. Your job is to gather nutrition information from a willing person and assess their nutritional status as if you were going to develop a nutrition plan. You may not choose a child (younger than 18 years of age) or yourself. You will not share the name of the volunteer on the form.
