CC (chief complaint): “I feel so worthless. I don’t like to leave the house because I don’t want to interact or associate with anyone, I have no interest in doing anything.”

HPI: P. A. is a 39-year-old African American male who presented to clinic for major depressive disorder (MDD). The patient is not currently taking any medications and has not tried any medications before. The patient reported lack of motivation, crying, lack of energy, and unwillingness to leave the house. The patient said her symptoms started 3 months ago after his daughter drowned in their house swimming pool.

Psychosocial History: The patient is a 36-year-old male. Patient having only one child which was the drowned daughter and the incident occurred in Jan 2024. Both parents have passed away. Mother died in 2020, father died in 2007. He has 2 sisters. Reports that he sleeps 4-5 hours per night and says appetite is fine. The patient had graduated from high school and said he was “college educated.” Patient reported that he was a financial manager in his job but lost it because he couldn’t focus at work after his daughter’s death.
