Completing and polishing my research

I am writing a research about gaming crisis 1983. 

I wrote the rough draft and I got it back with some notes, 

I was wondering if someone can help me finishing the whole thing . 

So basically what you need to know, it should be in MLA format. 

What do I need : 

1- cover page

2- Fixing grammar mistakes

3- 3 more sources ( 2 should be primary) 

4- from the sources you need to add more points and paragraphs. 

5- I will attach the Research Portfolio Requirements that my teacher want me to follow, so if you can have a quick look on that too. Like "annotated bibliography" I didn't do that either.
