• The nature of the attack: ransomware, DDoS, etc.
• The immediate impact on the organization: shutdown, degraded operation, etc.
• Whether the organization had a COOP to implement, and how the implementation went.
• How long did recovery take?  Is it still going on?  If so, how long since the attack?
• Did the institution survive the attack?
