Community Intervention

Each individual will propose a self-sustaining, social innovation project to address an identified need. The proposed project must be no more than 10 pages and include the following: 

(1) a statement of need of a community or neighborhood that includes current data 

(2) a recommended intervention with goals and objectives to respond to the community issue using an Evidence Based Practice (EBP) 

(3) a logic model; 

(4) a work plan/Gantt chart; and 

(5) a project budget of no more than $100,000.
