Group cognitive behavioral therapy involves a clinician working with multiple unrelated participants simultaneously, who shares similar challenges. It provides unique mechanisms like peer support, normalization of symptoms, and social skills practice. It is also cost effective, because it can treat multiple patients at the same time (Guo et al., 2021). It also presents challenges, such as maintaining confidentiality, given that multiple participants are sharing and sensitive information. In addition, it is difficult to balance the diverse needs and dynamics within the group, providing equal opportunities to participate, encouraging the shy ones, and maintaining the dominating personalities in check (Greysten et al., 2024).
Individual CBT, on the other hand, provides a private environment tailored entirely to a single client's specific cognitive schemas and core beliefs. It allows for precise personalization, and in deep exploration of personal trauma. Dynamics occur only between clients and therapists, ensuring patients’ privacy and undivided attention. However, it may be difficult to establish a trust relationship that allows clients to be vulnerable and receptive to cognitive restructuring (Van Eik et al., 2025). In the video showcasing individual CBT (MedCircle, 2019), the therapist and the client find his core belief by using a laddering technique and then explore his rules to communicate needs and wants based on that core belief.
The family CBT focuses on the relational dynamics and communication patterns within a family system, addressing how family members inadvertently reinforce behaviors or maintain symptoms. It works by altering mutually influencing thoughts and behaviors to create a calmer home environment. In the video regarding couples CBT, the therapist explains that it is different because it allows the participants to work on their different perspectives and interpretations on the other’s behaviors, practice techniques, empathy and assertiveness (Beck, 2018).
The application of this modality may be challenging, because family members frequently resist altering entrenched interaction styles, deflecting accountability or blaming other relatives for system dysfunction. It may be difficult to balance competing agendas, having to manage individual cognitive distortions and multi-person relational conflicts and shared systemic goals simultaneously (Varshini et al., 2025).
