Implementing Evidence in clinical practice
1-Discuss how you intend to implement your evidence-based findings
A multimodal approach is required to manage hypertension in the homeless population. Community outreach programs that include partnerships with local healthcare groups, non-profits, and community clinics to set up frequent health check-up camps for the homeless will be critical. These camps will provide vital services such as blood pressure checks and hypertension education. Furthermore, using mobile health units equipped with basic healthcare facilities will provide access to areas with high populations of homeless people (Vedanthan et al., 2019). These mobile clinics can serve as roaming health stations, giving frequent check-ups, prescription distribution, and health education. Furthermore, through forming alliances with homeless shelters and support organizations, hypertension screenings and educational programs will be integrated into current services, ensuring a more complete and long-term approach to healthcare delivery.
2-What are your anticipated challenges?
Several challenges may occur while putting these plans into action. Limited medical services are a significant issues because homeless people may encounter financial constrains, transportation issues, or a lack of knowledge about available treatments. Medication adherence is another issue, especially given the insecurity of living situations (Bedmar et al., 2022). Building trust and overcoming the stigma associated with health care settings poses additional challenges, as homeless people may have negative perceptions or previous negative experiences with healthcare practitioners.
3- How do you intend to overcome some of those challenges?
The process of addressing these concerns requires the establishment of mobile clinics and street teams capable of directly reaching regions with high homeless populations, as well as resolving transportation and accessibility issues. A comprehensive healthcare strategy will include collaboration with social workers, psychologists, and support agencies to address broader issues contributing to homelessness, increasing trust and involvement in healthcare services (Embleton et al., 2021). Educational programs will empower homeless people by teaching them the importance of hypertension management, the dangers of uncontrolled hypertension, and practical medication adherence measures. Ensuring culturally competent treatment through healthcare practitioner training will be critical in identifying and addressing the homeless community's unique difficulties. Furthermore, integrating the homeless community into the start and implementation of healthcare initiatives would cultivate a sense of ownership, therefore making programs more acceptable and effective.
