PLEASE read chapters 11 & 12 and respond to the following questions:

1. Using the internet (e.g., scholar.google.com), textbook, or other print book resources at your disposal, make a list of at least five scales/instruments that you might be able to use in your program evaluation proposal.

2. Describe each scale or instrument – description of scale, full item statement, norm scores, scoring, reliability, validity, interpretation, and reference – which should be matched with the program's desired outcomes WHICH are outlined in your Logic Model (e.g.,  depression, well-being, self-esteem, substance abuse, health-related quality of life, emotional and behavioral disorders, etc.)

3. Develop a questionnaire with a set of at least 20 or more items (using only nominal or ordinal or interval levels of measurement) to measure the socio-demographic information of clients or participants in your program/service for the evaluation project.
