1. Research the illness, list symptoms, medical treatment (including pharmacology), and prognosis of illness and statistics as they relate to the general population. R esearch should include a minimum of (3) references ( Do not use Internet websites or class text) . (2 pgs.)
1. Choose an OT model/FOR based on the psychological theory you have chosen. Explain how this model/FOR addresses your client’s limitations.
· i.e., MOHO: your client’s limitations are in the areas of volition, habituation, performance capacity; Development of adaptive skills: your client’s limitations are in the subsystems (sensory integration, cognitive skills, group interaction).
1. List 3 referral sources for services from which the client would benefit. (i.e., community; additional therapies, specialties). Indicate why this patient needs these particular referrals. (1 page)
· Please remember, your client is currently receiving services (psychiatrist, OT, etc., you are not going to replace these services, you are to recommend others that may aid the client in being successful, or helpful once discharged).

· Each STG must be able to be obtained within a one-hour group treatment.

· Justify the activity you have selected as being relevant to address your patient’s needs and level of function.
· Justify activity as being based on evidence to address STG
1. For each of the 2 STG’s establish medical necessity.
· Explain why this patient’s health would be compromised and/or how their functional independence would be affected if they do not receive this one-hour occupation-based treatment to address STG.
